Kneading Therapy is committed to protecting the privacy and confidentiality of client information in accordance with the Personal Health Information Protection Act (PHIPA) and other applicable Ontario legislation.
Information Collected
We collect personal and health information necessary to provide massage therapy services, including:
- Name, address, telephone number, and email address;
- Health history, presenting concerns, and treatment notes;
- Insurance information (if applicable); and
- Appointment scheduling and billing records.
Use of Information
Personal health information is used for:
- Providing massage therapy treatment;
- Scheduling and confirming appointments;
- Processing payments and insurance claims;
- Complying with professional and regulatory requirements (including CMTO standards); and
- Maintaining accurate clinical records.
Safeguards
Reasonable physical, technical, and administrative safeguards are in place to protect personal health information from unauthorized access, use, disclosure, or loss. Paper records are stored in a secured location. Electronic records are maintained with access controls appropriate to the sensitivity of the information.
Disclosure
Personal health information will not be disclosed to third parties without consent, except as required or permitted by law (for example, to comply with a court order or to prevent serious harm).
Retention
Records are retained in accordance with the requirements of the College of Massage Therapists of Ontario and applicable legislation.
Access and Correction
Clients have the right to request access to their personal health information and to request corrections. Such requests should be made in writing to Monique Cox.
